Documentation of behavioral services should capture where the client started and how behavior changes over time. This means including an assessment to determine baseline function and initial targets, and ongoing records showing progress toward those behavior-change goals. Together, assessment and demonstrated behavior change provide a complete picture of both the starting point and the outcomes of the intervention, which is essential for guiding treatment and accountability.

Describing interventions is important, but it’s part of how services are delivered rather than the core documentation focus. Records of interactions with clients are too general and don’t explicitly reflect the client’s progress. A summary of analyzed data is a component of the ongoing review, but on its own it doesn’t establish the baseline and the observable changes over time.
